If you are unable to complete the on-line application, please download the printable version (8 pages). We recommend that you then return to a computer and enter your information using the on-line application process. This will ensure that your information is accurate and complete. However, if you are unable to enter the information on-line, please mail the hard copy to our office along with the items described above.

While the Med-Start Office will accept applications via US mail, we strongly encourage you to submit your application using the on-line application process.
